Yes, you can be seen in clinic if you do not have insurance. There is no charge for you to see a provider. We only charge students for labs, medications given from our dispensary, and any procedures we may perform. We do our best to keep these costs low.

We do not accept any insurance other than the Student Health Insurance Plan. Everyone else must pay the self-pay fee for services at the time of the visit. Your insurance plan may reimburse what you have paid. We can provide you a list of charges to provide to your insurance carrier.

Yes. We can call in a prescription, provide a handwritten prescription, or send an electronic prescription to the pharmacy of your choice. However, we will only prescribe you medication after you have been evaluated by a provider.

Our immunization staff is available to help you with any immunization needs you may have. They are only available by email at[emailprotected].

Contact us at the clinic location your provider works or have your pharmacy send an electronic refill request. Please provide us at least 24 hours after receipt of this request to respond.

If you're enrolled in online classes only, have a religious objection to immunizations, or have a medical exemption, you can find exemption forms on the Patient Portal under "Downloadable Forms".

If you have a DD214 form, go to the Forms & Immunization Requirement tab and upload your form along with any immunization records.

Please remember that the "Distance Learning" exemption is only valid for one semester at a time, so you'll need to reupload the form each semester you plan on taking online classes instead of fulfilling immunization requirements.

Additionally, Religious Exemption forms require notarization by an official notary.

Keeping track of vaccination or immunization records is important as it shows a comprehensive history of all the vaccines one has received. Such records may be necessary for certain job applications, traveling to another country, or registering for school.

If you are a student and would like to request a copy of your immunization records, please visit https://health.gsu.edu/policies-forms/. Select Authorization for Release of Medical Information, fill out the form, and send it via email to [emailprotected]. The records can be mailed or picked up from the clinic during the weekdays, from 9 am to 5 pm.

You can upload your records to the patient portal by going to Forms & Immunization Requirements to meet the immunization requirements. You can upload a document or picture and include the dates when you received the vaccines or blood tests (titers).

To make an appointment in the patient portal, log onto the portal using your Georgia State username and password. Once logged in, go to the Menu Bar, and select "Appts." Next, select your MOST URGENT symptom and "Submit." Select your preferred Campus location, the Appointment Reason that best identifies with your symptom or Telehealth to get a virtual doctor's visit, and your preferred Provider. To see all the available appointments for every Provider at your preferred location, select "All" as the Provider and then "Submit."" Select your preferred date in the calendar and preferred time. Once you have selected your appointment time, you will receive additional instructions regarding your appointment and any forms that need to be completed before your visit. We highly encourage you to Opt-In to our Mobile Notifications to get updates and appointment reminders regarding your appointment.

Please note that appointments cannot be scheduled within two hours of the appointment time, so please plan accordingly.

Forms can be submitted to the patient portal by selecting “Upload” from the menu bar after logging onto your account. Select the document you are uploading from the drop-down menu and attach the file you wish to upload.

No. Transfer students will need to contact their former institution to obtain copies of their immunization records. Once students have obtained their copies they will need to upload their records to the patient portal. Students who do not meet Georgia State immunization requirements will have to take necessary vaccines or show proof of immunity to comply with the vaccination policy.

    • The first dose of these vaccines must be given at least 4 days before your 1st birthday or later and the second dose must be spaced at least 30 days after the first dose.
    • A positive blood titer test that proves that you have sufficient levels of antibodies to these viruses or a history of the disease can also be uploaded to satisfy the immunization requirement.
  • An up-to-date TDAP or TD vaccine(from within the past 10 years). As an example, if you received your last Tetanus vaccine in 2012 or earlier, it is currently expired, and you will need to receive a booster.
  • If you were 18 or youngerat your time of enrollment (regardless of how old you are currently), 3 doses of the Hepatitis B vaccine are required. (The second dose must be at least 30 days from your first, and the third dose must be at least 4 months after).

To make a Telehealth appointment, you would follow the same steps in scheduling a regular appointment and select Telehealth as your appointment reason when you get to the screen "Search for Next Available Appointments" screen.

Yes, the charge will post to your account as a "Student Health Clinic" charge, but it will not show as an itemized list of what services you were given at the time of your visit.

  2. Online Cohort Immunization Holds: If you are taking online classesonly, please log in to the patient portal. Choose Downloadable Formsthen choose "Immunization Waiver- Distance Learning Exemption."Please note that the “Distance Learning” exemption is on a semester-to-semester basis and will need to be reuploaded each additional semester that you plan on taking classes online instead of satisfying immunization requirements. Those who are are switching from online-only to in-person classes must submit their updated immunization records to be able to register for in-person classes.
  3. Other Holds:If your registration hold is not immunizations related, please contact GSU Admissions office at 404-413-2000 for further help.

GSU Immunizations office will not be able to verify immunization data. Students' records will be incomplete, and a hold will be placed on their student account, which will block them from registering for classes.

Students can be seen at any campus that is convenient to them. We have clinics on the Atlanta, Clarkston, and Dunwoody campuses. We also offer telehealth so you can be seen at any clinic.

It is required that students residing in campus housing receive the meningococcal conjugate vaccine.

Immunization exceptions (waivers) are customized accordingly for religious, military and medical reasons with proper documentation. Online or distance learning students are not required to provide their immunization records unless they are spending time on campus.

Students enrolled in classes and who have paid the health fee are eligible to be seen in the clinic.
